Membership Plans

Patients who elect not to participate in a membership program are still welcome to be seen at TCIM and at our sister clinic Abundant Health (phone number (951) 331-4020). You will be assigned to a new-hire advanced practitioner (FNP or PA) who will be trained in primary care but will likely not have extensive functional medicine training. Should you decide not to continue your care with TCIM or Abundant Health, we are committed to ensuring a smooth transition to your new primary care provider. You will enjoy: • Referrals for integrative and functional medicine treatments at TCIM for Chiropractic, Acupuncture, and IV Therapy • Annual Executive Physical • Portal access for messaging will be limited to simple requests such as medication refills and appointment requests that will be handled by the medical assistant but there will not be direct messaging to the provider. • There will be temporary access to your current provider until their membership panels are full, but membership patients will have priority access to scheduling and appointment times

Yes, everyone will have the same options under this new membership plan.

You may schedule an appointment immediately with a non-membership FNP or PA. However, if you wish to schedule with your current PCP, appointments may be limited. If no appointments are available, you will then only be able to schedule with a non-membership FNP or PA.

Yes. However, each provider is seeing a limited number of patients, so signing up sooner than later is very important.

Billing is on the 1st of the month, starting January 1, 2026. Even if you sign up immediately, billing will not start until January 1, 2026.

We do ask for a 12 month commitment. In doing so this keeps your spot yours with the provider of choice. Should you wish to cancel your membership during that time you will be dropped from the provider panel and open it up to a patient on providers waitlist.

17 years and below will be charged $25 a month if parents are membership patients at TCIM, IF not child will be charged provider regular price.

Yes, and you will still be able to see our non- membership NP/PA. However, if our membership providers’ panels get full you may lose access to them in the future.

We ask for a 30 day notice. You must log on to your Hint Health profile to cancel. If you cancel within the 12 month agreement you will be removed from the provider panel to open for another person on the waitlist.

You can change your membership one time during a calendar year.

Yes, patients with qualifying insurance plans or self-pay patients, the monthly membership fees apply.

No, membership fees are not covered by nor billable to insurance.

Please direct billing, pricing and insurance questions to our billing department. Lab test fee questions may be directed to our care team, however, patients should contact their insurance directly regarding lab coverage.

Yes. Your insurance will be billed for services at TCIM exactly as they always have.

Example 1: If you have a qualifying PPO Insurance or Medicare plan and choose Dr. Vellinga Basic functional membership plan: You pay a monthly membership fee of $59 not covered by insurance nor can it be used towards deductibles. Visits during the calendar year are as follows: $59 monthly membership fee + copay per visit with Dr. Vellinga Example 2: If you are a cash patient and want to see Dr Lundquist for a 30 minute appointment and are on Premium Functional Medicine Membership You pay monthly membership fee of $249 + visit fee of $200
